he belly isn't the only part of your body subject to middle-age spread. As we get older, the years of walking and standing can cause feet to spread and flatten out as ligaments that support the arch lose their holding power. The result: the condition known as fallen arches, or flat feet. "Keep in mind that we're talking about 26 bones in the foot that are supported by a series of ligaments, muscle tendons and other connective tissue," says Glenn Gastwirth, D.P.M., deputy executive director of the American Podiatric Medicine Association in Bethesda, Maryland. "Over a period of time, the ligaments stretch out or 'give' under the pressure of your weight-especially if you're overweight. So what usually happens is you go shopping for new shoes one day and suddenly realize that you now need a size 8 when you've always worn a size 6. You may think your foot has grown, but what really happens is that the foot has spread out, both in width and length."
Fallen arches in adults are caused by several things. Below are some of the most common causes. Abnormalities present from birth. Torn or stretched tendons (resulting from foot injuries or foot strains). Inflammation or damage of the PTT (posterior tibial tendon). The PTT is responsible for connecting the middle of the arch to the ankle and lower leg. Dislocated or broken bones (also as a result of injury). Health problems like rheumatoid arthritis. Nerve problems. Other factors like diabetes, obesity, aging and pregnancy (these factors are known to increase the risk of fallen arches).
Some people have fallen arches, and they aren?t even aware of it, fallen arches are sometimes asymptomatic and do not always cause pain. However, for others, the following symptoms may be present. Foot pain, particularly in the arches or heels, leg or back pain, feet feel tired quickly, swelling in the feet and difficulty moving the feet.
Your doctor examines your feet to determine two things, whether you have flat feet and the cause or causes. An exam may include the following steps, Checking your health history for evidence of illnesses or injuries that could be linked to flat feet or fallen arches, Looking at the soles of your shoes for unusual wear patterns, Observing the feet and legs as you stand and do simple movements, such as raising up on your toes, Testing the strength of muscles and tendons, including other tendons in the feet and legs, such as the Achilles tendon or the posterior tibial tendon, Taking X-rays or an MRI of your feet.
Non Surgical Treatment
Orthotics. Interpod orthotics re-align and support the foot; therefore reducing any excessive stress when walking or during activity. Orthotics can assist with maintaining arch profile and allow for more effective functioning of joints. Footwear. A strong supportive, well fitted shoe may assist with reducing excessive pronation and support the joints of your feet. A supportive shoe will also help maximise the function of your Interpod orthotic. Padding may be applied to your shoes or feet by your practitioner to reduce excessive stress. Specific taping techniques can be applied by your practitioner to improve foot function. Your practitioner may advise certain stretches or exercises to assist with maintaining foot function and reduce painful symptoms. Pain medication such as NSAIDs (ibuprofen) may be advised by your practitioner. If all conservative options have been exhausted, then surgical correction of flat feet may be undertaken.
Feet that do not respond to the treatments above may need surgery. The surgery will help to create a supportive arch.
Well-fitted shoes with good arch support may help prevent flat feet. Maintaining a healthy weight may also lower wear and tear on the arches.
Time off work depends on the type of work as well as the surgical procedures performed. . A patient will be required to be non-weight bearing in a cast or splint and use crutches for four to twelve weeks. Usually a patient can return to work in one to two weeks if they are able to work while seated. If a person's job requires standing and walking, return to work may take several weeks. Complete recovery may take six months to a full year. Complications can occur as with all surgeries, but are minimized by strictly following your surgeon's post-operative instructions. The main complications include infection, bone that is slow to heal or does not heal, progression or reoccurrence of deformity, a stiff foot, and the need for further surgery. Many of the above complications can be avoided by only putting weight on the operative foot when allowed by your surgeon.
